[Page] PT Help Me Grow Home Visitor – Board of DD Essential Board Duties and Responsibilities include the following: * Deliver evidence-informed home visiting services according to ODH and Healthy Families America (HFA) policies and procedures. * Deliver parent education curriculum to eligible families. * Provide direct in-home, intensive home visiting services. * Demonstrate excellent interpersonal skills, communication skills and collaborative skills. * Participate in child find/outreach activities with local agencies and attend collaborative meetings as assigned. * Conduct home visits and secure reports and other pertinent information from agencies and professionals involved in delivering services. * Establish eligibility for HMG Home Visiting services. * Develop Family Goal Plans with family participation. * Encourage and foster family independence in decision making regarding needed services and supports. * Develop and maintain individual case files/case notes for all referrals and eligible individuals in a confidential and up to date manner and system. * Schedule and conduct required home visits for families in accordance with local and ODH rules, Healthy Families America Best Practice Standards. * Attend and participate in requires trainings and meetings. * Enter, maintain and update information as required into ODH OCHIDS data base system. * Initiate and facilitate local/ODH requires transition activities for children turning three (3) years of age or relocating. * Fulfill the duties of home visitor as required by ODH and HFA policy and procedure. * Fulfill the duties of parent educator as required by according to curriculum standards. * Gather information, interview families, coordinate/complete screenings, evaluations, ongoing assessments and Family Plans. * Assists families and children in group settings as appropriate to ensure health and safety. * Provides exemplary customer service through courteous, responsive interactions with Board staff, community partners, and individuals and families receiving services. * Adheres to Board policies and procedures. * Understands and practices professional ethics in keeping the confidentiality of information and materials with which he/she may come in contact with. * The forgoing is not intended to represent an exclusive listing of job duties and work characteristics included within the position.
