As of June 15, 2018, due to restrictions by the U.S. Passport Agency, the Clerk of Court's office will no longer process U.S. Passport Books and/or U.S. Passport Cards.
E-filing for Criminal, Domestic, and Court of Appeals will begin on May 28, 2019, with CourtView's Justicefiling.

The passport application forms are also available in the Clerk of Courts office. You can print the application below. Pages 1-4 are instructions for the applicant and should be read before completing the Application for a U.S. Passport.

The following forms are available for printing. Failure to use the correct form will result in a delay in the issuance of your passport:

DS-11: Application for Passport Book or Card

This form must be presented and signed in person at an authorized passport acceptance facility which is the Erie County Clerk of Courts Office or the U.S. Post Office.

You must use this form IF:

  1. You have never had a U.S. passport,
  2. You are under age 16,


  3. If your most recent passport was lost or stolen, (and you are applying because your passport was lost or stolen) you will also need to submit a Form DS-64.
